29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On August 16, 2016 at the jobsite located at or near 9559 N Hunter St in Liberty, MO employees were exposed to a struck by hazard in that they were using pneumatic nail guns without eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On August 16, 2016 at the jobsite located at or near 9559 N Hunter St in Liberty, MO, employees were exposed to a fall hazard in that they were working on a surface approximately 11 feet above the ground with no fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On August 16, 2016 at the jobsite located at or near 9559 N Hunter St in Liberty, MO employees were exposed to a fall hazard in that the ladder they were using to access a roof did not extend 3 feet above the landing surface.
