29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about May 6, 2021, and at times prior thereto, employees were working in a construction area without the use of hardhats where overhead hazards exist, exposing employees to struck by hazards.

29 CFR  1926.452(j)(2):Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.  On or about May 6, 2021, employees were working from a Louisville pump jack that was not properly anchored at heights above 10 feet, exposing employees to a fall hazard.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  On or about May 6, 2021, and at times prior thereto, employees were installing exterior siding on residential townhomes at heights above 6 feet without the use of fall protection, exposing employees to a fall hazard.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about May 6, 2021, and at times prior thereto, employees were accessing the roof of residential town-homes by means of a ladder that did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard.
