29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, shall be 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level. When conditions warrant, the height of the top edge may exceed the 45-inch height, provided the guardrail system meets all other criteria of this paragraph.   On or about August 16, 2016, at the above addressed jobsite, an employee was performing residential construction operations next to the unguarded window openings on the second floor, and was not protected from falling, thereby exposing the employee to fall hazards of approximately 14.2 feet.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about August 16, 2016 at the above addressed jobsite, an employee performing residential construction operations on the trusses, over 25 feet above the ground, and was not protected from falling, thereby exposing an employee to fall hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  Kingdom Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1141491, citation number 1, item number 1  and was affirmed as a final order on June 7, 2016 with respect to a workplace located at 16 N. Ewing Ave., Naperville, IL 60540.
