29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) On or about July 17, 2017 - at the above addressed jobsite - employees performing framing operations on a job made wooden scaffold did not have an appropriate ladder to safely access the work zone, thereby exposing the employees to falls of approximately eight (8) feet four (4) inches.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) On or about July 17, 2017 - at the above addressed jobsite - employees performing residential carpentry operations on the roof were not protected from fall hazards, thereby exposing the employees to falls of approximately eleven (11) feet six (6) inches. b) On or about July 17, 2017 - at the above addressed jobsite - employees performing residential carpentry operations inside unit 10731 were not protected from fall hazards, thereby exposing the employees to falls of approximately eight (8) feet nine (9) inches. The Kingdom Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1171425, citation number 2, item number 1 and was affirmed as a final order on December 02, 2016, with respect to a workplace located at 712 Wellner Road, Naperville, IL 60540. The Kingdom Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1141491, citation number 1, item number 1 and was affirmed as a final order on June 07, 2016, with respect to a workplace located at 16 N.
