29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles: (a) Western States Fire Protection at 2501 Dallas Street, Denver, CO 80010: On and before August 17th, 2016, the employer did not ensure that the points of operations on the heavy-duty deep-cut variable-speed band saw (port-o-band), Model # D28770, S/N 030410 200650, were properly guarded to prevent employees from having any part of their body in the danger zone during operation. Operators placed their hands within one inch of the point of operation while holding material during cutting operations. This hazard exposed the employees to an amputation hazard.
