29 CFR 1910.212(a)(3)(ii): The point of operation machine guards were not designed or constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about 08/15/16 an employee engaged in cutting tubing in the Tubing Department was exposed to amputation hazards. The employee was exposed in that the guard over the guillotine cutter was not constructed to keep the employee from inserting a finger past the guard.
