On or about August 23, 2106, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about August 23, 2106, at the above addressed jobsite, an employee performing roofing operations approximately twenty two (22') feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards. The Jimmy's Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1078837, citation number 1, item number 1 and was affirmed as a final order on September 16, 2015, with respect to a workplace located at 4458 Gilbert Avenue, Western Springs, IL 60558. The Jimmy's Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 945196, citation number 1, item number 2 and was affirmed as a final order on January 7, 2014, with respect to a workplace located at 240 Plainfield Road, Countryside, IL 60525.
