29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about August 19, 2020 employees performing roofing operations on a flat roof were not protected from a fall, exposing the employees to a fall of 11.4 feet.    Jimmy's Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1172012, citation number 2, item number 1 and was affirmed as a final order on November 17, 2016, with respect to a workplace located at 310 Stone Lagrange, Illinois 60527.   Jimmy's Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1078837, citation number 1, item number 1 and was affirmed as a final order on September 16, 2015, with respect to a workplace located at 4458 Gilbert Avenue, Western Springs, IL 60558.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 19, 2020 employees used an extension ladder that did not extend three feet past the upper landing, exposing the employees to the hazards associated with a fall.  Jimmy's Quality Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1) which was contained in OSHA inspection number 1172012, citation number 1, item number 1 and was affirmed as a final order on November 17, 2016, with respect to a workplace located at 310 Stone Lagrange, Illinois 60527.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d)
