29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about 8/12/2016 an employee had her right forearm and hand caught in a paddle mixer during cleaning activities when another employee turned on the equipment. The equipment involved in the incident was a Rietz Blender, Model RS-28-K5206, Serial No. RS-740136, on Line #1, which was not locked out during the activity. The employer failed to implement policies during this cleaning activity for the following requirements outlined in the standard; 29CFR 1910.147(d)(3)- Isolating energy sources from the machine 29CFR 1910.147(d)(4)- Application of Lockout devices 29CFR 1910.147(d)(6)- Energy isolation verification
