OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause serious physical harm to employees, in that employees were exposed to falls to lower levels from on a pallet supported by a power industrial truck:    During the period between July 17th, 2016, and August 17th, 2016, at Heritage Tile employees used a forklift with a single pallet on the load engaging means (forks) to lift personnel in the air as high ceiling height to change light bulbs on the ceiling of the warehouse and shipping room. They were also elevated into the air on a pallet to wrap an extension cord around the ceiling mounted heater.

29 CFR 1910.36(g)(4):   Objects that project into the exit route must not reduce the width of the exit route to less than the minimum width requirements for exit routes:    On or about August 17th, 2016, at Heritage Tile there were two exits blocked. The east exit route was reduced to 13"  by pallets of materials. The west exit route was reduced 27.5" by a table with boxes on it.
