Employees were permitted to operate the forklift in that the nameplate, which identifies the load capacity, model number and serial number, was not legible; and the lever control indicators worn out.  b) Employees engaged in pallet jack operation and material handling activities in Plant 8 by the Decanting area were exposed to caught-in hazards. Employees were permitted to operate the 16702-9 South-Worth Stack-N-Go battery powered motorized (electric) pallet jack - green machine; model #4439390; serial #71100-300-D4/130349 in that the guard/shield was broken and missing for the shroud surrounding the back guide wheel.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
