29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition: Employees engaged in forklift operation and material handling activities, using the Yale G158 forklift, in Plant 8 by the Decanting area, were exposed to struck-by hazards in that the nameplate, which identifies the load capacity, model number, and serial number, was not legible; and the lever control indicators worn out. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Employees engaged in forklift operation and material handling activities, using the Yale G158 forklift, in Plant 8 by the Decanting area, were exposed to struck-by hazards. Employees were permitted to operate the forklift in that the nameplate, which identifies the load capacity, model number and serial number, was not legible; and the lever control indicators worn out. b) Employees engaged in pallet jack operation and material handling activities in Plant 8 by the Decanting area were exposed to caught-in hazards. Employees were permitted to operate the 16702-9 South-Worth Stack-N-Go battery powered motorized (electric) pallet jack - green machine; model #4439390; serial #71100-300-D4/130349 in that the guard/shield was broken and missing for the shroud surrounding the back guide wheel. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
