29 CFR 1926.502(d)(9):  Lanyards and vertical lifelines did not have a minimum breaking strength of 5,000 pounds:    (a) Custom Solar at 505 27th Way, Creekside Apartments, Boulder, CO 80305: On and before August 22nd, 2016, the employer did not ensure that each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Employees were installing solar panels on the carport roof without the use of fall protection.  Condition exposed employees to fall hazards of 11 feet.
