29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: 1) On August 20, 2016, and at prior times, L&L Design Builders, Inc. did not ensure that its employees were protected from falls, in that a mobile scaffold had a wheel with a missing lock and two wheels with locks that did not function.
