29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 24, 2016, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances: 1) In the tube mill department, a tube cut-off machine being operated without proper blade guard provided to protect employees from contacting the blade while in operation. Employees were exposed to possible amputation and crushing hazards.

29 CFR 1910.252(a)(2)(vii): Where relocation of combustible was impracticable, cutting or welding was permitted when combustible were not protected with flame-proofed covers or otherwise shielded with metal, asbestos guards, or curtains: On or about August 24, 2016, and at times prior thereto, in the precut area an employee was allowed to perform cutting, grinding and welding operation within 35 feet from combustible materials without flame proofed protection provided. The employees were exposed to fire hazard.
