On or about August 4, 2016, at the turbine bay, surfaces which include, but are not limited to the following, were not maintained as free as practicable of accumulations of lead: - Locker 33 at riverside wall by turbine bay 3 - Vacuum near locker 33 - Electrical box near upstream wall by turbine bay 3 c. On or about August 4, 2016, at the stairs between the generator bay and the control room, surfaces which include, but are not limited to the top of electrical MCC 8P, was not maintained as free as practicable of accumulations of lead. d. On or about August 4, 2016, by generator 2, surfaces which include, but are not limited to the following, were not maintained as free as practicable of accumulations of lead: - Top surface of tan cabinet - Floor under speed increaser - Floor near access to generator 2 pit e. On or about August 5, 2016, equipment gallery, surfaces which include, but are not limited to horizontal pipes and the floor were not maintained as free as practicable of accumulations of lead.
