29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    On or about 04 February 2016, the Employers written lockout/tagout program did not include procedures that clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of unexpected energizing, start up or release of stored energy. Employees gaining access to the machines such as the CNC's, Lathes, drill presses, and Bridgeport Mills who perform servicing, maintenance, and cleaning operations were exposed to caught in hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. 29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    On or about 30 August 2016, the employer failed to install and use equipment in accordance with the instructions listed and permitted uses. Numerous knockout type boxes were installed as pendant extension cords and used in the production area. Employees were exposed to a shock hazard. 29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    On or about 30 August 2016, the employer did not develop, implement, and maintain a written hazard communication program at the workplace which described how the criteria specified in 1910.1200(f), (g), and (h) will be met for employees exposed to hazardous chemicals, including propane, hydraulic oil, kerosene, alcohol, adhesives, and cleaning chemicals.
