29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employee(s) from caught in hazards at the facility. This was most recently documented on September 21, 2016 at 1011 Ellison Ave Omaha, NE 68110. This hazard(s) observed in the following but not limited to: a. The Soffit Shear located on the South Wall of the facility had an opening in the existing shroud/guard of 8 inches that allowed for a person to be able to reach into point of operation. b. On or about August 24, 2016 an employee's thumb was amputated when reaching for a tape measure located adjacent to a pinch point of the shear. At the time of the incident a guard had been removed exposing the pinch point. This pinch point in conjunction with the sharp edge of the tape measure caused the amputation.
