An employee sustained an amputation to the right thumb as a result. 2) Building 2, Press Brake #2 - An employee was allowed to operate a Cincinnati Press Brake #2 that was not equipped with any safeguarding, such as but not limited to two-hand controls devices, presence sensing device, pull-back device an/or restraint device. Employees were exposed to caught-in hazards.
