29 CFR 1910.95(d)(1): A representative monitoring program was not developed and implemented when information indicated that an employee's exposure may equal or exceed an 8-hour time-weighted average of 85 dBA: The employer failed to conduct initial monitoring to determine if employees were exposed to noise level in excess of the permissible exposure level of an 8 hour time weighted average of 85 dBA. a) Plant 2- Press # 27: A press operator was exposed to continuous noise levels of 69.8 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA). The equivalent dBA level was approximately 87.4 decibels (dBA). A sampling survey was conducted on September 27, 2016 for approximately 384 minutes. A zero time exposure was assumed for the period of time that was not sampled. b) Plant 2- Press #20: A press operator was exposed to continuous noise levels of 76.8 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA). The equivalent dBA level was approximately 88.0 decibels (dBA). A sampling survey was conducted on September 27, 2016 for approximately 382 minutes. A zero time exposure was assumed for the period of time that was not sampled. c) Plant 1- Press #11: A press operator was exposed to continuous noise levels of 73.8 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA). 29 CFR 1910.95(g)(1): The employer shall establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels. The employer failed to establish an audiometric testing program that included providing annual audiometric testing for employees exposed to noise level in excess of the permissible exposure level of an 8 hour time weighted average of 85 dBA. a) Plant 2- Press # 27: A press operator was exposed to continuous noise levels of 69.8 % dose of the permissible 8-hour time weighted average of 85 decibels (dBA).
