The flexible electrical extension cord was frayed at the cord cap.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    A.) C & M Painting Company, Inc., 701 Bayfront Parkway, Erie, PA, 16507, north side of the building, on or about August 30, 2016 and times prior thereto:  Employees were working from a tubular welded frame scaffold to perform work operations including, but not limited to, installing foam boards for stucco work.  Employees were accessing a fabricated frame scaffold via an attachable ladder.  Upon stepping off of the ladder and onto the scaffold, the employees were stepping onto a section of scaffolding that was not fully decked.  There was a 26-inch gap between the scaffold upright and the first accessible plank.

The employees were exposed to a fall hazard of 14.2-feet.  A guardrail system was not installed on the outrigger work platform or the end frame.  B.) C & M Painting Company, Inc., 701 Bayfront Parkway, Erie, PA, 16507, north side of the building, on or about August 30, 2016 and times prior thereto:  Employees were working from a tubular welded frame scaffold to perform work operations including, but not limited to, installing foam boards for stucco work. Employees were accessing a tubular welded frame scaffold via an attachable ladder at a height of 13.4-feet.  Guardrails were not installed on the east side end frame located at the 2nd tier near the access ladder.  C.) C & M Painting Company, Inc., 701 Bayfront Parkway, Erie, PA, 16507, north side of the building, on or about August 30, 2016 and times prior thereto:  Employees were working from a tubular welded frame scaffold to perform work operations including, but not limited to, installing foam boards for stucco work. Employees were accessing a tubular welded frame scaffold via an attachable ladder at a height of 13.4-feet.  A mid-rail  and or equivalent members were not installed on the north side 2nd tier at the left side of the access ladder.
