29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about August 13, 2016, the employer failed to report to OSHA an amputation of an employee's left index finger tip. The employer has failed to report the amputation to OSHA within 24 hours. This most recent occurrence of this took place at the facility located at 6002 N. 89th Cir, Omaha, NE 68134 where on August 13, 2016 the an employees finger left index finger tip was amputated after it was crushed by an equipment part to the Lokotrack LT1213 concrete crusher. The employer did not report the incident to OSHA.
