29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  a. On or about April 3, 2019, on the 1:12 pitch 240-foot x 60-foot metal roof 7 employees were walking and working on the roof removing old and installing new metal roof panels with an unprotected skylight, without proper use of complete fall protection systems to protect from falling through the uncovered and unguarded plastic skylight panel flush with the roof, being exposed to 32-foot fall hazard.  b. On or about April 3, 2019, on the 1:12 pitch 240-foot x 60-foot metal roof 7 employees were walking and working on the roof removing old and installing new metal roof panels, without proper use of complete fall protection systems to protect from falling through the uncovered and unguarded approximately 74-foot length x 2-foot wide opening at top of roof ridge, being exposed to 32-foot fall hazard.  c. On or about April 3, 2019, on the 1:12 pitch, 240-foot x 60-foot metal roof, 7 employees were walking and working on the roof removing old and installing new metal roof panels, without use of any fall protection systems to include a complete personal fall protection/restraint system to protect from falling at the unprotected roof edges, being exposed to 30-foot along east roof edge and 15-foot 6-inch along west roof edge fall hazards.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a. On or about April 3, 2019, the employer had 1 of 3 safety ropes that were connected to anchors and available for employee use, that had worn damaged fibers and defective safety latches that would not close back when released, that had not been removed from service.
