OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, including severe burns, to employees in that employees were potentially exposed to dust explosion, deflagration, or other fire hazards from dust collectors used at the facility: a) Employees were potentially exposed to fire and explosion hazards from dust collectors located inside the building. The two AAF and one FARR material separators located in the mixer area were used to collect plastic dust. The dust collectors were not equipped with any devices to mitigate a potential dust explosion and/or fire, on or about, September 1, 2016. b) Employees were potentially exposed to fire hazards from dust collector located inside the building. The Foremost material separator located in the re-grind department was used to collect plastic dust and was not equipped with any devices to prevent foreign materials from entering the regrind operation, on or about, September 1, 2016 c) Employees were potentially exposed to fire hazards from the dust collector located inside the building. The Foremost material separator located in the re-grind department was used to collect plastic dust and was not equipped with any devices to mitigate a dust fire, on or about, September 1, 2016. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
