29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about June 22, 2017, the employer did not ensure hazardous energy control procedures were utilized for the safe application, usage, and removal of energy controls when employees access the door to grease fittings and other maintenance work on the bagging and finishing drier unit. Energy Sources include, but are not limited to, electrical including rotating belts and pulleys.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about June 22, 2017, employees were exposed to machine hazards associated with electrical and gravity as sources of energy. The employer failed to implement energy control application steps as the bagging and finishing department drier was not shut down or turned off to perform maintenance and servicing work. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)] and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.22(d)(2): Hazardous conditions were not immediately repaired or properly guarded before an employee used the walking-working surface: On or about June 22, 2017 employees were exposed to tripping hazards when the employer failed to properly guard a floor opening created by a discharge pipe in the floor. The pipe created an opening of approximately 4 inches wide by 48 inches length next to the walkway and employees were required by job necessity to step over the pipe to set the drier speed. OMNOVA Solutions Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(a)(8)(ii), which was contained in OSHA inspection number 936706, citation number 01, item number 001 and was affirmed as a final order on November 6, 2013 , with respect to a workplace located at 1380 Tech Way Drive, Akron, OH 44306.
