29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a/ Inside of the Fabrication Area, the barrier guard belonging to the Accurshear, m/n 625010, s/n 5775, was improperly positioned thereby exposing the operator to a crushing hazard posed by 15 stock holders.

This machine caused an amputation injury on or about 08/29/16. b/ Accurshear, m/n 625010, s/n 5775 (front and rear).
