(a) On or about April 01, 2024, and at times prior, the employer failed to address defects and safety issues, such as the absence of windshields or back shields for Doosan Forklift Model: 705-5. (b) On or about April 01, 2024, and at times prior, the employer failed to address defects and safety issues, such as non-operational horn for Toyota Forklift Truck Model: 7FDU45.

29 CFR 1910.178(q)(7): Industrial trucks were not examined at least daily and after each shift before being placed in service, and were placed in service when examination showed a condition which adversely affected the safety of the vehicle: (a) On or about April 01, 2024, and at times prior, employees were exposed to struck by hazards while operating the Doosan Forklift Model: 705-5 that was not inspected before each shift daily prior to use. (b) On or about April 01, 2024, and at times prior, employees were exposed to struck by hazards while operating the Toyota Forklift Truck Model: 7FDU45 that was not inspected before each shift daily prior to use.
