29 CFR 1910.106(e)(8):  "Repairs to equipment." Hot work, such as use of spark-producing power tools was permitted without supervision of an individual in responsible charge; and/or inspection of the area to be sure that it was safe for the work to be done and that safe procedures were followed for the work specified.   a) On or about 08/31/16 in the shop area; employees were exposed to a thermal burn hazard when they used an air powered grinding wheel to cut the Victaulic clamp bolts on the gasoline pipeline for tanker truck # 3440 without implementation of a safety procedure, such as but not limited to, (SPES - Truck/Service Vehicle Garaging Checklist) to ensure the line was fully purged of flammable liquids prior to beginning the use of spark producing tools.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  a) On or about 09/06/16 in the shop area; employer provided fire extinguishers and expected employees to use them on incipient fires but did not include this subject as part of their initial employment training program conducted on or about 07/26/16 exposing employees to burn hazards while performing work on or in proximity to cutting/grinding  ( spark producing ) operations around flammable/combustible liquids.    ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment:  a) On or about 09/06/16 in the shop area; employees used hazardous chemicals, such as but not limited to, Well Worth brand Shop Kleen and Slippery Stuff ( flammable aerosols ), Safety Kleen Solvent ( combustible ), and were not given information and training on these and other materials in the shop area at the time of their initial assignment on 07/26/16.   ABATEMENT CERTIFICATION REQUIRED
