29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points: On or about August 29, 2016 maintenance employees were expected to access the electrical cabinets that serviced the AIDA Caseline machine and were exposed to hazards that related to exiting the building. The employer did not ensure that exit pathways were maintained at 28 inches or more at all times; the exit paths from the electrical cabinets were approximately 16 inches at one end and 21 inches at the other end.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: On or about August 29, 2016 employees required to access and service the AIDA endwall press electrical cabinet were exposed to electrical injuries and the rear cabinet doors were blocked by a guardrail that had been installed approximately 12.75 inches away.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the working space did not permit at least a 90-degree opening of equipment doors or hinged panels: On or about August 29, 2016 employees required to access and service the AIDA endwall press electrical cabinet were exposed to a shock hazard and the rear cabinet doors were blocked by a guardrail and did not allow the doors to open at least 90 degrees.
