29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about 08/25/2016, Doors and Windows area - employees were exposed to an amputation hazard while operating the Paslod component nailer machine (model CM2, serial # C2A1509), in that, the guards were missing. An employee suffered an amputation on his left hand pinkie finger when the finger was caught between a metal clamp and the wood boards. b. On or about 08/31/2016, Doors and Windows area - employees were exposed to an amputation hazard while operating the Paslod component nailer machine (model CM2, serial # C2A1509), in that, the employer permitted employees to operate the machine without guards.
