29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: a) On August 26, 2016, the employer did not ensure the lock out tag out procedures for the 25402 molding machine provided specific requirements for verifying the rotating cutters have stopped prior to performing work. An employee suffered a partial finger amputation when making an adjustment to the cutter.
