Hand feeding tools are not a point of operation guard or protection device and shall not be used in lieu of the "guards" or devices required in this section: a. On or about September 2nd, 2016, at the above addressed jobsite, in the manufacturing area, an employee was exposed to an amputation hazard, while working with a press, and placing and removing materials without hand tools.

On or about September 2nd, 2016, at the above addressed jobsite, in the manufacturing area, an employee did not receive the proper supervision while operating a power press. The employee was exposed to an amputation hazard when placing and removing materials in the point of operation without using hand tools.
