29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a. On or about August 3, 2016, at the above addressed jobsite, employees conducting carpentry operations on the second floor, ten (10') feet above a lower level, were exposed to falls created by the presence of unprotected floor holes.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On or about August 3, 2016, at the above addressed jobsite, employees performing carpentry operations on the second floor were exposed to fall hazards of approximately ten (10) feet above a lower level created by the presence of unprotected sides. A & M Construction & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1000132, citation number 1, item number 1 and was affirmed as a final order on December 29, 2014 , with respect to a workplace located at 2331 Parkside Ave., Chicago, IL, 60639.
