29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  a) At the Serrano Construction two story apartments project, the yellow extension cord used for hand tools had damaged outer insulation and the outdoor power station had broken cover.  The Serrano Construction & Development was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.404(b)(1)(iii)(C), which was contained in OSHA inspection number 958718.015, citation number 1, item number 2 and was affirmed as a final order on 12/19/2014, with respect to a workplace located at Naval Base, Santa Rita, Guam, 96915.
