29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Location - 130 Crescent Avenue, Chelsea, MA: On or about 8/31/2016, the Capway Systems oven on Bake 2 was not powered down prior to opening a panel to clean a filter. (b) Location - 309 Andover Street, Lawrence, MA: On or about 10/25/2016, the unguarded transfer pump used in conjunction with the slurry mixer was not powered down prior to inspecting it for cleanliness.
