29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about September 8, 2016, at Lot 63, 599 Loretta Way, Grayson, Georgia:   An employee performing framing work was exposed to struck-by hazards from falling nail guns, and plywood from the employees working above him when he worked from the ground without wearing a hard hat.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 8-9, 2016:  a) Two employees performing framing work on Lot 65, 579 Loretta Way, Grayson, Georgia, were exposed to fall hazards of approximately 18.5-feet to greater than 25-feet when they worked from the trusses and at the unprotected edges of a house without using fall protection.   b) Six employees performing framing work on Lot 63, 599 Loretta Way, Grayson, Georgia, were exposed to fall hazards of approximately 13.3-feet to approximately 20.8-feet when they worked at the unprotected edges of a house without using fall protection.   c) Six employees performing framing work on Lot 63, 599 Loretta Way, Grayson, Georgia, were exposed to fall hazards of approximately 20.8-feet and greater when they worked within 6-feet of an unprotected floor opening without using fall protection.   d) An employee performing framing work on Lot 62, 609 Loretta Way, Grayson, Georgia, was exposed to a fall hazard of approximately 11.1-feet when he worked from the top plate of a house without using fall protection.   e) An employee performing framing work on Lot 59, 297 Dunaway Court, Grayson, Georgia, was exposed to a fall hazard of approximately 11.7-feet when he worked from the top plate of a house without using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 8-9, 2016, at Lot 63, 599 Loretta Way, Grayson, Georgia:  Six employees performing framing work were exposed to a fall hazard of approximately 11.1-feet when they used a 20-ft aluminum extension ladder that was not extended at least 3-feet above the second floor landing.
