29 CFR 1910.24(h): Stair railings and handrails were not installed in accordance with 29 CFR 1910.23:  Basement Stairs:  On, or about, September 12, 2016, the wooden stairs leading from the main level to the Basement Press Room did not meet construction requirements in that:  A.  The top rail was measured to be 27 inches above the nose of the tread. B.  The top rail was not the equivalent of 2 x 4 inch stock. C.  A midrail was not provided. 29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):  Basement Press Room:  On, or about, August 22, 2016, the following mechanical power presses were not provided with adequate safeguarding for the point of operation:  A.  Perkins Model 450-B:  The Woodbury pullback device was missing several components and was not usable.  An employee suffered serious injuries when his hand was caught in the die while stamping parts.  B.  Perkins Model 351-B:  The Woodbury pullback device was missing several components and was not usable.  C.  Excelsior bench press:  The Woodbury pullback device was not used when the press was used to stamp end plates.  Also, no one at the company was trained regarding the proper set-up of the device and the device was not routinely inspected.
