29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Jobsite: On or about September 13, 2016 employees working on the ground were not wearing hard hats and were exposed to the hazard of falling objects such as hand tools and roofing materials used by employees working on the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) Jobsite: On or about September 13, 2016 employees were exposed to a fall hazard of approximately nine (9) feet while working on a roof without the use of conventional or alternative forms of fall protection. Ivan Paredes, dba Lincoln Construction (currently, Lincoln Master Construction, Inc.) was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection numbers 994865, 986765, & 314920877, citation number 1 & 2, item number 1 and was affirmed as a final order on 12/16/14, 12/16/14, & 4/4/12, with respect to a workplace located at 49 Elmcrest Ave., Providence RI, 223 Oliphant Lane, Middletown RI, & 321 Plainfield St., Providence RI.
