29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) Jobsite:  On or about 10/20/2016, eye and face protective equipment was not used when employees were using a pneumatic nail gun to fasten roof shingles as well as a leaf blower to blow roofing debris off the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a) Jobsite:  On or about 10/20/2016, employees were not protected from falling while working on a roof at levels of approximately sixteen and twenty-seven feet above a lower level.    Ivan Paredes, dba Lincoln Construction (currently, Lincoln Master Construction, Inc.) was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection numbers 994865, 986765, & 314920877, citation number 1 & 2, item number 1 and was affirmed as a final order on 12/16/14, 12/16/14, & 4/4/12, with respect to a workplace located at 49 Elmcrest Ave., Providence RI, 223 Oliphant Lane, Middletown RI, & 321 Plainfield St., Providence RI.
