Condition observed on or about 09/13/16.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a- Side Of Building, 97 Isabelle Court, Marlton NJ: A worker was exposed to fall hazards in excess of 25 feet to the ground surface when using/working from a pump jack scaffold whose walking/working surface was missing end stops and mid-rails. This employee had not received scaffold hazard recognition training from a competent person at the time of or prior to the worksite inspection.  Condition observed on or about 09/13/16.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a- Front Of Building, 97 Isabelle Court, Marlton NJ: Workers were exposed to fall hazards in excess of 25 feet to the ground surface when using/working from aluminum extension ladders, that were missing feet, contained visibly damaged rungs, contained visibly bent side rails, were missing the rope used to extend the fly section, etc. These ladders had not been inspected by a competent person prior to being used.  Condition observed on or about 09/13/16.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  a- Front Of Building, 97 Isabelle Court, Marlton NJ: Workers were exposed to fall hazards in excess of 25 feet to the ground surface when using/working from aluminum extension ladders, that were missing feet, contained visibly damaged rungs, contained visibly bent side rails, were missing the rope used to extend the fly section, etc. The employees utilizing these ladders had not received ladder hazard awareness or/and minimization training from the company at the time of or prior to the date of the worksite inspection.
