An employee was observed using a pneumatic nail gun without any form of eye or face protection while installing roofing material.

Employees were observed installing roofing materials on a 4:12 - 7:12 pitched duplex residential roof at heights greater than 6 feet above the ground below.

The employer is failing to ensure that all employees are protected from fall hazards when entering/exiting the landing surface of a residential roof. Employees were observed using ladders that did not have side rails which extended to at least 3 feet above the landing surface when entering/exiting the roof to remove roofing materials.
