29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 130 Park Avenue, Florham Park, NJ. Worker was exposed to 11 to 12 foot falls to the ground below from the platform of the fabricated frame scaffold as when his lanyard was anchored to the scaffold's end frame, the worker's fall protection system was rigged such that he could fall 18. 5 feet meaning that he would hit the ground before his fall protection system could work. Violation observed on or about 09/13/2016. b) 130 Park Avenue, Florham Park, NJ. Workers were exposed to 16 foot falls to the ground below from the top of the columns as with their lanyards anchored to the rebar at the top of the forms, the workers' fall protection systems were rigged such that they could fall 18.5 feet meaning that they would hit the ground before their fall protection systems could work. Violation observed on or about 09/09/2016.
