29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about September 14 and 15, 2016 at a worksite located at or near 2845 Plateau Dr, Conway, AR 72032, the general contractor did not ensure that the framing subcontractor protected employees from fall hazards while working on a roof top that was approximately 25ft above lower levels. b. On or about September 15, 2016 at a worksite located at or near 2810 Plateau Dr, Conway, AR 72032, the general contractor did not ensure that the guttering subcontractor protected employees from fall hazards while installing gutters on a roof edge that was approximately 15 feet above lower levels. c. On or about September 15, 2016 at a worksite located at or near 2740 Plateau Dr, Conway, AR 72032, the general contractor did not ensure that the siding subcontractor protected employees from fall hazards while installing sidings on a roof edge that was approximately 13 feet above lower levels.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a. On or about September 14, 2016 at a worksite located at or near 2845 Plateau Dr, Conway, AR 72032, the general contractor did not prevent subcontract employees sheathing the roof of a residential building from being exposed to fall from height hazards when mounting and dismounting a ladder that was not extended at least 3 feet above the upper landing surface. b. On or about September 15, 2016 at a residential construction worksite located at or near 2810 Plateau Dr, Conway, AR 72032, the general contractor did not prevent subcontract employees from being exposed to fall from height hazards when ascending and descending to and from a roof on a stepladder that was not extended at least 3 feet above the upper landing surface. c. On or about September 15, 2016 at a worksite located at or near 2740 Plateau Dr, Conway, AR 72032, the general contractor did not ensure that the subcontractor did not expose employees to fall from height hazards when mounting and dismounting a ladder that was not extended at least 3 feet above the upper landing surface.
