29 CFR 1910.147(c)(4)(i): Procedures were not developed and documented for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   A)    Whirlpool Corporation, located at 1300 Marion-Agosta Road, Marion, OH 43302: On or about September 16, 2016, the employer did not develop specific procedures to control hazardous energy of the equipment such as, but not limited to, the Whiting 30-Ton overhead crane (Serial # 12171), located in the West Bay of the Press Room. The crane had multiple disconnects for electrical energy. Servicing contract employees, conducting maintenance activities on the crane, were not provided with specific procedures how to control hazardous energy sources of the Whiting 30-Ton overhead crane. Servicing contract employees were exposed to struck-by and fall hazards in a case of unexpected energization of the crane while contract employees were conducting work.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:  A)    Whirlpool Corporation, located at 1300 Marion-Agosta Road, Marion, OH 43302: On or about September 16, 2016, the employer did not inform the outside servicing contract employees of the companys procedures for the control of hazardous energy sources while the servicing contract employees were conducting servicing and repairs of the equipment such as, but not limited to, the Whiting 30-Ton overhead crane (Serial # 12171), located in the West Bay of the Press Room. The employer did not verify that the outside contract employer has adequate program and outside servicing contract employees have received required training for the control of hazardous energy sources. Servicing contract employees were exposed to struck-by and fall hazards in a case of unexpected energization of the crane while contract employees were conducting work.
