29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:  Instances: 1) On or about 9/19/2016, at the above address jobsite education department office E1-024, the floor was wet therefore exposing employees to a slip and fall hazard.  2) On or about 9/19/2016, at the above address jobsite Biscayne unit (FCI), the floor was wet therefore exposing employees to a slip and fall hazard.  3) On or about 01/25/2017, at the above address jobsite Special Housing Unit (SHU), a section of the ceiling was leaking, exposing employees to a slip and fall hazard.

On or about 9/27/16, at the above address jobsite CMS area east side, the stairways did not have a uniform combination of risers while having an angle of 45 degrees with risers that ranged from 6-7.5 inches.
