OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by a wire rope. On or about 21 September 2016, and at times prior thereto, two wire rope clamps installed on a cable and chain assembly for a motorized pulley to move a platform in and out of the media blast room within Building 1 were oriented incorrectly, exposing employees working in and around the area to the hazard of being struck by the wire rope from a whipping effect. Additionally, the amount of turn-back rope on the opposite end for the 3/8ths inch cable was less than the required 6.5 inches and had a single clamp at the time of the inspection. The platform is used to move large steel pieces which can weigh up to approximately 15k pounds or 7.5 tons.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch. On or about 21 September 2016, and at time prior thereto, two pedestal bench grinders were being used in which the tongue guard was not adjusted to a maximum of 1/4th inch from the surface of the abrasive wheel, exposing the employees to the hazard of being struck by the wheel fragments in the event of the wheel disintegrating. 1) Dayton 10 Inch Bench Grinder located in Building 1 had a gap of approximately 1.5 inches from the tongue guard to the surface of the wheel. 2) Bench grinder located in Building 2 had a gap of approximately 1.5 inches from the tongue guard to the surface of the wheel.
