29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it is lifted more than a few inches. On or about January 31, 2017, and in times prior thereto, a round piece of metal over 7 feet in diameter was being lifted with a two part sling using hooks depending on friction alone to be held in place. Employees were exposed to the hazards of being struck by the load in the event of an unbalance load and hooks slipping out.

On or about January 31, 2017, and in times prior thereto, a round piece of metal over 7 feet in diameter was being lifted with a two part sling using hooks and a flat metal bar that depended on friction alone to be held in place. Employees were exposed to the hazards of being struck by the load in the event of the hooks and metal bar slipping out.
