29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 09/19/16, Letter Shop Department, 5200 East Lake Blvd, Birmingham, Alabama, 35217, the employer did not utilize energy control procedures while by-passing the interlocks on the MSE Mail Stream Inserter during change over and paper jam removal exposing the employees to pinch hazards.
