On or about March 28, 2018, the employer did not ensure procedures were developed, documented, and/or utilized for the safe application, usage, and removal of potentially hazardous energy controls while clearing jams and setting up (service and maintenance activity) the Kolbus Perfect Binding Machine.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about March 28, 2018, the employer did not ensure binding machine operators authorized to perform service and maintenance activities, such as blade changes, setups, and clearing paper jams, on the Kolbus Perfect Binding machine received training in the recognition of hazardous energy sources, the type and magnitude of the energy available, and the methods and means necessary for energy isolation and control.
