29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: (a) GR Construction, LLC at 2733 W. 28th Ave, Denver, CO: On or before September 21, 2016, the employer used a Gehl all-terrain forklift, model number DL8H, to elevate two employees. While installing windows, the two employees stood on and worked from a scaffold of plywood sheeting that was affixed to the forks and carriage of the forklift and not designed by a qualified person. This condition exposed the two employees to a fall hazard of approximately 26 feet.
