29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with the top rail:  a/  The two stair rails belonging to a flight of stairs leading up to the Line Sorter were missing intermediate rails.  The flight of metal stairs consisted of 12 risers.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation (P.O.O.) of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a/  Inside of the Production Area, the P.O.O. belonging to the American Baler Company Horizontal Baler, Predator III, s/n 8133-E, was inadequately guarded.  Specifically, the opening of the hopper measured approximately 58"W X 35"L X 11"D.  The slide or ram moved beneath the opening forming a gap that measured approximately 58"W X 3/4"H.  The operator placed her right hand into the hopper where it became caught in this opening and was severely crushed on or about 09/26/16.        ISI Environmental& Recycling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 905192, citation number 1, item number 1, and was affirmed as a final order on 08/27/13, with respect to a workplace located at 1210 Commercial Avenue, Cairo, IL 62914.
